Please email, fax or bring the form with you to your next appointment.
Otherwise, you can fill in the form online, using the ”Online – Patient Registration Form” directly on the VOA website.
Consultation Fees
The VOA is not a bulk billing practice and therefore you will be out of pocket for your consultation. Payment is required in full at the time of your consultation and we can electronically claim the Medicare rebatable portion on your behalf at the time of your appointment.
At the time of booking your appointment we will provide you with our Practice Information which will discuss our fee structure further.
For Workcover and TAC patients with a valid claim number we will send the invoice for your consultation directly to your Workcover insurer or TAC. For those patients without a valid claim number at the time of the appointment we request that you pay your account in full at the time of consultation and claim back yourself through your Workcover insurer or TAC.
DVA Gold Card patients will not be out of pocket for any portion of their treatment pathway with our surgeons. All invoices, consulting and surgical will be sent directly to the Department of Veterans Affairs.
Surgical Fees
Once your surgical pathway has been determined by you and your surgeon and you wish to book in for your operation, we will provide you with a written quote outlining the fees for your surgical procedure along with admission, fasting and registration procedures.
For uninsured patients who wish to have their surgery through the private system we will obtain quotes from all parties associated with your procedure and provide fee estimates to you. On acceptance of these fee estimates we will book your surgical procedure at a mutually convenient time for you. Please note all expenses for your procedure will need to be paid in full prior to the surgery taking place.
For Workcover and TAC patients requiring a surgical procedure we will write to the relevant Workcover insurer or TAC to obtain acceptance of liability for your surgical procedure. There will be no out of pocket costs for you once we have received acceptance of liability.
For DVA Gold Card holders your surgery costs will be fully covered by the Department of Veterans Affairs.
